Prepare for Personal Taxes

Use this checklist in order to prepare your Personal Income Tax information. We have included telephone numbers or website links to sources:

  • Have all of your Income information available. This can include the following:
    • Employment – T4 Slips (Contact your employer).
    • Other Income or Pension¬† – T4A Slips (Contact your payer).
    • Employment Insurance (or repayment) – T4E Slips (Contact Service Canada 1-800-206-7218).
    • WCB or Social Assistance Benefits – T5007 Slips (Phone WCB 604-214-5411 or Social Assistance 1-888-922-8804).
    • Canada Pension/Old Age Security – T4AP and T4OAS Slips (Call Service Canada at 1-800-277-9914).
    • RRSP Income – T4RSP Slips (Call your bank or investment adviser).
    • RRIF Income – T4RIF Slips (Phone your bank or investment adviser).
    • Universal Child Tax Benefit – RC62 slip (Call Canada Revenue Agency at 1-800-387-1193).
    • Investment Income – T3 / T5 / T5013 Slips (Phone your bank or investment adviser).
    • Capital Gains/Losses – T1170, T3, T5008, T5013 , T4PS Slips (Phone your bank or investment adviser). If you keep track of your gains and losses manually click here for the capital gain/loss form.
    • Support Payment received. Have payer name and amount received (note that only the taxable component of the support payments received are taxable. Non taxable child support payments received are not taxable).
    • Rental IncomeClick here for the rental income form.

If you are missing any of the above slips, contact the provider and ask for the slip name (T3, T4 etc) and then the box numbers and value contained. We can use this information in replacement of the actual slip.

  • Have all of your Deductions information available:
    • Pension Adjustment – T4 or T4A Slips (Phone your employer).
    • RRSP Contributions – RRSP, T4 or T4A Slips (Contact your provider). Note that you can include any contributions from January 01 and for 16 months forward to February 28th.
    • Union Dues – Union Notice or T4 slips (Phone your employer or Union Office).
    • Universal Child Care Benefit repayment – RC62 Slips (Contact Call Canada Revenue Agency at 1-800-387-1193).
    • Child Care expenses –¬† Have the child care provider name (and Social Insurance Number if individual) and amount paid for the year.
    • Business Investment Loss – Provide details.
    • Moving ExpensesClick here for the Moving Form.

    • Clergy Residence Deduction – Form T1223 – Click here for Form.
    • Other Deductions – Provide Details.
  • Don’t forget about the available Tax Credits:
    • Spousal Claim – We strongly suggest you have our office prepare your spouses return to ensure all available deductions and credits are used.
    • Children and Dependents – Have the full name and birth dates. This can include infirm dependents whom you care for such as elderly parents. In this case we strongly suggest preparation of all returns for children and dependents.
    • Public Transit Passes – Include a total of monthly (or longer) bus passes for public transit. Separate by each recipient.
    • Children Arts Amount. Have the total (separated by each child) up to $500 for registration or membership in a prescribed program or artistic, cultural, recreation or development activities.
    • Children Fitness Amount. Have the total (separated by each child) up to $1,000 for registration or membership in a physical activity.
    • Caregiver Amount – Have the caregiver information available. We strongly recommend that we prepare the caregiver tax return to ensure all available deductions and credits are included.
    • Tuition (for self or dependents – we strongly recommend we prepare dependents tax return)- Have the approved T2202 Slip (payment receipts are not acceptable) from the school. You can obtain this electronically from most post secondary schools, colleges and universities:
    • Interest Paid on Student Loans – (Contact your bank)
    • Disability Amount – Notify us or provide the completed T2201 form – click to access form.
    • Medical Expenses – Provide totals for self, spouse and each dependent. Click here for a list of approved medical expenses.
